Payment by cash, check, HSA/FSA debit card, or major credit card is accepted.  Payment is due prior to, or at the time of service.

Dr. Zaki accepts Lyra Health benefits. 

Although she is not presently in-network for other insurance, she can assist you in obtaining partial reimbursement for your sessions if your insurance carrier povides out-of-network benefits.  You may receive from between 20-80% of the fee reimbursed.  Before your first session, you should contact your insurance carrier to verify whether you have out-of-network mental health benefits, the number of sessions covered, whether teletherapy is covered (if applicable), the rate of reimbursement, and how much of a deductible may need to be met.  If you have questions about this, Dr. Zaki is available to assist.


Good Faith Estimate

You have the right to receive a “Good Faith Estimate” explaining how much your medical and mental health care will cost. Under Federal law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical services, including psychotherapy services. You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency services, including psychotherapy services. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule a service. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.
